(AGING) CAP/DA Case Manager - Social Worker

PAY GRADE: 69

SALARY: Begins at $42,060 with complete benefits package

DESCRIPTION OF WORK: The primary purpose of this position is to provide advanced human support work involving the assessment of client needs and access to appropriate services, case management and client advocacy. Considerable knowledge of public assistance programs, socio-economic impact issues and individual and group dynamics is required. Strong organization, communication, and problem-solving skills along with thorough demonstrated knowledge of office procedures and associated equipment are inherent to the position.

QUALIFICATIONS AND EXPERIENCE: A bachelor's degree in social work or closely related field and moderate experience in case management and social work are minimal requirements.
