Meta Care Inc., a vibrant and forward-thinking healthcare organization, is looking for a compassionate and dedicated Member Ambassador to join our Austin, TX  In this PART TIME ROLE to possibly going FULL TIME you will help our members navigate the healthcare system, coordinate and schedule their medical appointments, and offer essential benefit support. We’re seeking someone who takes a proactive approach to member care, ensuring that each member has seamless access to the resources and services they need.

This role does not include hands-on clinical care or heavy lifting. Instead, it focuses on coordinating schedules for preventive care and supporting disease and chronic care management. It’s a distinctive, mission-driven position dedicated to providing essential support for those in our care.

Responsibilities:

  1.  Educate members about preventive care and wellness initiatives through outreach efforts.
  2.  Provide educational materials and resources to help members understand and access healthcare services.
  3. Schedule and coordinate a range of healthcare appointments, including medical, dental, and specialist visits.
  4. Assist members in setting up and using remote monitoring devices (e.g., glucometers, blood pressure cuffs, medical alert systems).
  5. Manage the delivery of device supplies and other essential health-related resources.
  6. Assess home safety and organize home modifications or meal services as needed.
  7. · Identify and coordinate community support services, such as transportation and home care, for members.
  8. Help members navigate benefit coordination and collaborate with healthcare plan design vendors.
  9. Develop and implement outreach campaigns to inform members about available benefits and services.
  10. Participate in client meetings to provide information on services and address any unmet needs.
  11. Follow up with members after hospital discharge and conduct on-site hospital visits for those who choose to participate.
  12. Work with company pharmacists and social workers to offer additional support to members.
  13. Maintain confidentiality and comply with PHI and HIPAA guidelines.
  14. Interact professionally and respectfully with members and colleagues.
  15. Travel to member locations and events as needed.
  16. Perform additional duties as assigned by the supervisor.

Requirements:

  • Minimum of 3 years of experience, preferably in a healthcare coordination or support role.
  • Excellent communication and interpersonal skills, with a strong ability to listen and provide emotional support.
  • Strong organizational skills, attention to detail, and the ability to effectively manage schedules and maintain accurate records.
  • Humble, personable demeanor with a genuine desire to assist and support others.
  • Ability to work independently as well as collaboratively with healthcare providers.
  • Proficiency in Microsoft Office products (Word, Excel, PowerPoint).
  • This position is suitable for someone who has years of experience in a medical office environment with a passion for coordinating and managing schedules and navigating for patients the healthcare maze.
  • For interested LPN or LVN’s this job does not require direct clinical patient care, so an inactive license is acceptable. This position would fit a nurse ready to move away from bedside care.
  • For interested CNA or MA’s an inactive certification is acceptable.

Compensation and Benefits:

· Salary Range: $18.00 to $24.00/ hour based on experience

· Comprehensive benefits package includes medical, dental, and vision insurance, retirement plans, and paid time off.

· Company will contribute 90% of individual medical health benefits

· Vacation/Sick Leave

· 7 Paid Holidays
