Key Responsibilities:
Helps to provide a comprehensive plan to meet clients needs, daily as well as long
term.
Maintains records of clientβs daily activities, including geheral condition of client.
Aids in clientβs daily routine, activities include but are not limited to, personal care, light
housekeeping, errands and meal preparations.
Accurately and timely completes aide activity sheets, submitting activity sheets on a
timely basis as specified by department policies.
Attends a minimum of 12 hours of annual in-service education.
Communicates any concerns/changes in client condition to appropriate staff in a
timely manner and documents on Aide Activity sheet.
Keeps family and staff informed of clients activities and concerns.
Completes annual skill competency program per NYS regulation
Performs other duties as required
