Licensed Social Worker MSW - Renal Dialysis - Full Time (1.0) - Day

Description: The Licensed Social Worker is responsible for providing social services for dialysis patients.  The SW will work in collaboration with other Renal Social Workers, Renal Dieticians, Nephrologists, and the patient to achieve high quality renal care; participate in Care Plan meetings with the patient and interdisciplinary care team; patient outcomes will be reported monthly at the Renal QAPI meeting, and complete other SW tasks as required. The SW will follow the ESRD CMS guidelines, dialysis program and MVHS policies and procedures. 

Requirements:

-Master’s Degree in Social Work

-Current NYS SW license

-Minimum 1 year SW experience- preferred

-Dialysis experience-preferred

-Professional appearance, positive attitude, and excellent attendance record.

-Excellent interpersonal and communication skills with ability to function in a collaborative fast paced environment

-Computer skills

-Cover multiple dialysis clinics as needed
