Job Description
  • To provide an evidence based occupational therapy (OT) service to palliative care patients in a variety of settings focussing on adaptation and rehabilitation.
  • To assist in the development and evaluation of the occupational therapy service.
  • To work autonomously with the St Clare Hospice team to contribute to exemplary care for patients and their families.
  • To be a resource and support to other Hospice staff and community professionals by offering OT knowledge and skills.


Requirements

  • Act as a Named Occupational therapist for identified patient(s) - ensuring all aspects of Care Planning are implemented and reviewed according to relevant protocols.
  • Ensure all reports and risk assessments are concise accurate and clearly documented in line with the Code of Ethics and Professional Conduct (College of Occupational Therapists 2010) relevant, clearly identified and risk management plans are clear and consistent.
  • Ensure that you make professional contributions to MDT decisions.
  • Ensure that you complete and contribute to formal risk assessment documentation.
  • Ensure that all standards of ward procedure are implemented to the highest standard
  • Attend and take part in Ward Rounds, CPA’s and other meetings as required.
  • Ensure correct procedures take place for the Admission and Discharge of Patients.
  • To deliver formal psychological interventions following appropriate training.
