JOB RESPONSIBILITIES

  • Medication Orders - Evaluates orders based on patient's diagnosis, weight, allergy history, and compatibilities. Properly enters orders in the patient profile. Dispenses medication orders accurately and prepares medication for delivery to the patient. Contacts the clinic’s physician or nurse as needed for order clarification. Detects errors within a medication order, works with clinics to correct these errors, and with these changes improves patient care. Participates in all areas of the medication-use system while offering individualized pharmaceutical care to patients.
  • Serve as a Resource -  Serves as a resource and liaison to our outpatient clinics. Uses the most up-to-date resources to ascertain correct and complete answers to medication related questions (examples include but are not limited to: drug compatibilities and interactions, appropriate administration of the medication, appropriate dosage form selection and dosing for patient weight, age, and diagnosis). Reviews patient information thoroughly to identify and alleviate drug-related problems, improper drug or dose selection, adverse drug reactions, drug interactions, medication use without an indication, and treatment failures. Handles questions and problems in a professional manner. Anticipates and analyzes conflict situations and takes preventative or corrective action with your supervisor. Identifies concerns within the department and assists in problem solving and decision making. Functions as an informational resource for our healthcare system. 
  • Pharmacy Operations: Works as an active member of the multidisciplinary team and collaborates with healthcare providers in our outpatient care areas to provide patient-centered care. Assesses patient-specific needs and considers age and disease specific characteristics in drug therapy education to our patients. Discusses evidence-based medication changes with the care team based upon patient response and provides clinical interventions to detect, alleviate, and prevent adverse drug events. Identifies high-risk patients and implements interventions to improve their quality of care and safe use of medication.
     

JOB QUALIFICATIONS

  • Bachelor of Science in Pharmacy OR PharmD Degree

  • Pharmacist License in the state of Ohio or license within 3 months of hire

Preferred

  • Prior experience with Specialty medications
