Project FIND’s mission is to provide low- and moderate-income and homeless seniors with the services and support they need to enrich their lives and live independently. Today Project FIND operates three supportive housing residences that are home to about 600 people and four senior centers with over 3,000 members. Our members and residents range from the healthy and active to the frail and homebound to the homeless. Job Summary:


The Social Worker will work with older adults at the Hamilton House location. The program provides case management services, information, referrals, supportive counseling, housing assistance, group work and crisis intervention for clients and their families.  The Social Worker is responsible for conducting detailed assessments to determine client needs and eligibility for services and engaging with clients to develop goal-oriented service plans. 


Responsibilities include:



  • Conduct intake and assessment screening, assessing mental and physical health needs and supports, and financial eligibility for benefits, entitlements; conduct home and office and community visits to address unmet needs. 

  • Implement constructive short and long-term interventions through counseling education about counseling and physical health supports, long-term planning, and assistance with benefits and entitlements. 

  • Provide orientation, support, and services necessary to assist tenants to remain permanently housed in a safe environment.   

  • In collaboration with Property Management, assist with income affidavits, housing applications and recertifications and assist in collecting rent as needed.  

  • Develop and create training events to educate clients on topics that can help support them in their daily lives (e.g., self-care, health and wellness, legal planning).



  • Lead one or more support groups, providing emotional support and psychoeducation about various issues related to aging in place, health and wellness, and self-care. 

  • Provide timely documentation and maintenance of client files and progress notes.

  • Provide supervision, training, and support to paraprofessional staff, including students.

  • Perform other related tasks as requested or assigned.




Qualifications:



  • Bachelor's degree in human services or related field required. MSW/LMSW preferred with at least 2 years relevant experience in social services; knowledge of benefits and entitlements a plus. 

  • Experience working with older adult populations and families preferred. 

  • Bilingual language Spanish fluency preferred.  

  • Must have strong computer skills, including Microsoft Office. 

  • Ability to provide accurate and timely documentation required. 

  • Time management skills to coordinate multiple responsibilities and services. 

  • Ability to work independently and as part of a team, in both a home and office environment. 

  • Maintain client and agency confidentiality. 

  • Excellent interpersonal, public speaking, and written communication skills. Must be able to communicate effectively and appropriately with other team members, community agencies, and clients with diverse opinions, values, characteristics, and cultures.  


Project FIND participates in E-Verify, which confirms employment authorization through an electronic database maintained by the Social Security Administration and Department of Homeland Security. The E-Verify process is completed in conjunction with the Form I-9 Employment Eligibility Verification.  E-Verify is not used as a tool to pre-screen candidates.
