Scope of Position

In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, family, pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy and students and other associate hospital personnelScreens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactionsParticipates in appropriate hospital and/or departmental committees relevant to practice area. 

 

Position Summary

This position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas. Duties include but are not limited to: medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nursesPatient population includes adult, adolescent, geriatric, and neonatal patients depending on the practice locationMust be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients servedMust be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and ageThis position is also responsible for supervising Pharm.D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy staff). 

Minimum Qualifications

Bachelor's Degree in Pharmacy. Registered Pharmacist. 0 years of relevant experience required. 0-2 years of relevant experience preferred.
