Description:
This position will be responsible for reviewing patient treatment plans and ensuring treatment guidelines are met, assisting with optimization of treatment plans and will support both our Precision Genomics and Research Programs. This incumbent will also partner with our Oral Oncolytic Nurse Navigator to provide coverage for our Oral Oncolytic Program, and provide drug information and education to patients and the healthcare team.
This position will service Ball Hospital, our clinic in New Castle and our clinic at Jay Hospital. Pending successful completion of training, this position offers a hybrid work schedule with two days on site (preferably Tuesdays and Wednesdays) with 3 days of remote work.
Demonstration of practice experience is required in one of three ways:
- At least four years of Oncology Pharmacy practice experience after licensure/registration as a pharmacist1 within the past seven years, with at least 50% of time spent in Oncology Pharmacy practice; or
- Successful completion of PGY1 pharmacy residency within the past seven years, plus at least two years of Oncology Pharmacy practice experience after licensure/registration as a pharmacist with at least 50% of time spent in Oncology Pharmacy practice; or
- Successful completion of PGY2 pharmacy residency in Oncology Pharmacy within the past seven years.
Board Certification is required with in 1 years of being hired into this role.
- Requires graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) or a program outside the U.S. that qualifies the individual to practice in the jurisdiction.
- Requires current Indiana Pharmacist License or eligibility for Indiana licensure as a pharmacist
