JOB SUMMARY:
As a ConcertoCare Paramedic, you will deliver in-home clinical care in collaboration with our clinical providers and care team to improve patient outcomes and prevent unnecessary ER visits and avoidable hospitalization.
Key Details:
- Location: Primarily field-based, conducting home visits.
- Reports to: Practice Manager with clinical oversight by the Market Medical Director
- Daily work: Facilitates telehealth visits for patients in their home, performs clinical skills and procedures, and conducts health screenings.
- Collaboration: Partners with providers, licensed practical nurses, nurse case managers, social workers, pharmacists, community health workers, and care navigators.
Key Responsibilities:
- Facilitate virtual visits in patients' homes to support care delivery under the guidance of a remote clinical practitioner.
- Perform clinical skills and procedures, including blood draws, laboratory specimen collection, point-of-care tests, medication and vaccine administration, basic wound care, basic hearing and vision tests, and ABI/QuantaFlo. All clinical skills require a competency evaluation and are carried out under the supervision of a clinical practitioner, with scope determined by state regulations.
- Conduct virtual visits with patients to complete health screenings and questionaries.
- Provide clinical support to providers and the interdisciplinary care team, including preparing charts, coordinating care after visits, and managing outbound calls and tasks as directed and supervised by an authorized healthcare practitioner.
- Process and track orders and referrals placed by our providers, working to ensure that patients complete orders and referrals in accordance with their treatment plan.
- Provide exceptional customer service to patients, caregivers, and clinical partners, ensuring proactive, timely, and comprehensive communication with a holistic, patient-centered approach.
- Contribute to initiatives aimed at improving patient engagement, experience, and health outcomes.
- Participate in team huddles and meetings to collaborate effectively with colleagues.
- Adhere to all applicable healthcare laws, regulations, and company standards
- Perform other duties as assigned.
QUALIFICATIONS:
- Active, unencumbered paramedic licensure and fulfillment of all state-specific requirements in the state you will practice.
- NREMT Paramedic certification preferred.
- Completion of a formal Paramedic training program or commensurate clinical experience.
- Basic life support (BLS) certification required.
- Preferred: 3 yearsβ experience.
- Preferred: Prior experience caring for patients in the home setting including private homes and/or congregate living facilities. Advanced training in complex care, geriatrics, and/or palliative medicine is an asset.
- Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
- DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No undisclosed violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless driving or felony conviction within the last 7 years.
Base Salary/Wage Range $27.00 to $30.00/hour plus annual bonus. Compensation for the role is commensurate with the candidateβs qualifications, skills, competencies, and experience and may fall outside of the range shown.
