Pharmacist on-call/per-diem

Winters Healthcare is on the lookout for a per-diem pharmacist to fill in for us during vacancies, around 2 times per month, with additional occasional coverage to cover for the occasional vacation and/or sick leave. 


As a Winters Healthcare pharmacist, you will be part of our integrated care team (medical, dental, and behavioral health integration) to deliver pharmaceutical care in an outpatient, patient centered community health center. The pharmacist is responsible for preparing and dispensing medication in a safe and effective manner, providing medication information and education to patients and prescribing providers. The pharmacist provides medication information and education in various settings such as amongst a clinical team, as well as one-on-one with Winters Healthcare patients, and other individuals who choose us as their pharmacy.


The ideal candidate is an experienced pharmacist, with proven ability to adapt to new workflows and systems redesign to best meet the patient need. In addition, the ideal pharmacist has experience or strong desire to work with primary care teams to support the overall health of our patients and community. Responsibilities:

  • Communicate, collaborate and work in a team-oriented environment supporting our model of care
  • Drug procurement, inventory control, board of pharmacy registration, delivery supervision and front-end management (supervise pharmacist(s) and pharmacy technicians).
  • Prepare and dispense medication in a safe and effective manner, providing education to patients and colleagues

Qualifications:

  • Bilingual fluency in English/Spanish is required (ability to read, write, and speak)
  • Current California Registered Pharmacist License
  • Certification from the North American Pharmacist Licensure Examination (NAPLEZ) and the CA practice standards and jurisprudence examination for pharmacists (CPJE)
  • Experience and ability to collaborate with other members of the patient care team to support patient health
  • Significant background providing outstanding customer service
  • Strong computer skills; knowledge of electronic health records eClinical Works and RX30 is preferred.

Hours:

  • Operating Hours: Mon., Tues. and Thurs.: 8:00 am – 8:00pm; Wed. and Fri.: 8:00 am – 5:00pm.
  • Working Hours: per-diem/on-call, with about 1-2 times per month of coverage 

Compensation and Benefits:

  • Hourly rate for the per-diem pharmacist is $65 - $80/ hour and is based on qualifications and experience. 
  • Paid Time Off (PTO accrues per hour worked). 
  • Malpractice liability insurance is provided.

Per Diem Pharmacist

Summary of Duties:

Under the supervision of the Lead Pharmacist, Pharmacist will work as part of an integrated team with medical, dental, and behavioral health providers to accurately prepare and dispense medication for patients in an integrated outpatient, patient-centered community health center as well as for other community members.  The Pharmacist manages medication protocols and will collaborate with the Lead Pharmacist and care teams to ensure appropriate, safe, and cost-efficient treatment needs and options are addressed for optimal patient care. The pharmacist prepares and dispenses medication to patients supporting medical, dental, and behavioral health personnel to provide consultation, information, and education on medication for patients

 

Essential Functions:

  • Perform functions as pharmacist, in compliance with Federal and CA pharmacy law, as well as DEA, confidentiality and HIPAA, 340B pharmacy regulations, and prescription drug coverage.
  • Ensure accurate prescription directions for use, age specific dosage, medication interactions, therapeutic duplications or overlaps, allergy information, and other pertinent data appropriate for individual patients
  • Maintains the security of drug inventory and pharmacy records
  • Provide medication information and education in various settings such as amongst a clinical team, as well as one-on-one with Winters Healthcare patients, and other individuals who choose us as their pharmacy
  • Administer immunizations as needed and advocate for patient safety and care
  • Evaluate patient medical and medication histories for drug-related problems and adjust drug therapy accordingly.
  • Responsible for completing a Drug Utilization Review (DUR) to ensure prescriptions are filled accurately and appropriately, following CA Pharmacy Board regulations.
  • Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
  • Identify opportunities to enhance patient care/cost savings
  • Review, check eligibility, and document patient encounters in electronic medical record (ECW and RX30)
  • Work with care teams and RN Care Coordinator to identify gaps in care and develop and implement protocols to improve overall patient health outcomes.
  • Advocate for patients with care teams, communicating on barriers to care, medication education, access to prescription medications, and work with care teams on patient’s willingness and ability in self-care.
  • Provide education on self-care and point of care devices, including proper use of glucometers and medication delivery devices to patients and care teams.
  • Participate in continuing education, staff development training, and attend staff meetings.
  • Support quality improvement efforts, and participate in related activities as required.

Minimum Requirements

Education, Certification/licensure and Work Experience

  • Doctor of Pharmacy Degree (PharmD) from an accredited School of Pharmacy or a Bachelor's degree with advanced clinical training experience required.
  • Minimum of 1 year of experience as a pharmacist
  • Current and active CA Registered Pharmacist license
  • Satisfactory completion of a Pharmacy Residency Program (PGY) or equivalent experience
  • CPR certification

Knowledge, Skills and Abilities:

  • Fluency (the ability to read, write and speak) in English and Spanish
  • Comprehensive knowledge and familiarity of all medications used within the scope of a pharmacist as defined by the CA Board of Pharmacy and Pharmacy Law, following current pharmacological principles
  • Significant experience and knowledge of the above mentioned essential functions
  • Knowledge of federal and California pharmacy law, as well as DEA, confidentiality and HIPAA, 340B pharmacy regulations, and prescription drug coverage.
  • Knowledge of eClinical Works (eCW), and RX30 Retail Prescription Processing Program preferred
  • Requires effective oral and written communication skills, excellent interpersonal skills, and computer literacy.
  • Ability and comfort building relationships with coworkers and patients
  • Knowledge of cultural competence and its potential impact on care of the patient
  • Significant background providing outstanding customer service
  • Well-organized and detail-oriented with the ability to establish and maintain working relationships with patients, employees and the public
  • Proficient in computer programs such as Microsoft Office: Word, Excel, Outlook
  • Previous experience with Federally Qualified Health Centers (FQHC), low income or medically underserved populations preferred, with a passion and dedication for serving the healthcare needs of our clients
  • Demonstrates ability to resolve drug distribution system issues.
  • Ability to exercise discretion to ensure patient confidentiality is maintained, and a professional, respectful environment exists at all times.
  • Ability to collaborate with a diverse group of individuals to convey medication information in a way that is accessible to understand and put into practice.

  • Monday, Tuesday and Thursday: 8:00 am – 8:00pm; Wednesday and Friday: 8:00 am – 5:00pm.

While performing the duties and tasks of this job, Employee is regularly required to stand; walk; work on irregular surfaces;  reach with hands and arms; use hands to finger, handle or feel objects, tools, or controls; talk or hear; and taste or smell. Employee is occasionally required to sit and stoop, kneel, crouch, bend, squat, twist or crawl. This job requires physical effort and the ability to place or retrieve items at below waist level may be required.

Employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.  Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision and depth perception, and the ability to adjust focus.  Employee occasionally may be required to transfer patients to/from a wheelchair. Environmental/ Working Conditions:

The environment is a standard pharmacy environment.  Regular office hours are Monday through Friday.
