The Pharmacist at Cross Road Health Ministries, a Federally Qualified Health Center (FQHC) clinic is responsible for providing comprehensive pharmaceutical care to patients. This includes dispensing medications, counseling patients on proper medication use, conducting medication therapy management (MTM), and collaborating with healthcare providers to optimize patient outcomes. The pharmacist ensures compliance with all regulatory requirements and contributes to the overall mission of the FQHC to provide high-quality, accessible healthcare to underserved populations.

Key Responsibilities:

  1. Medication Dispensing and Management:
    • Accurately fill and dispense prescriptions for patients.
    • Verify patient information and prescriptions for accuracy.
    • Maintain inventory of medications and supplies, ensuring proper storage and handling.
    • Perform medication therapy management (MTM) and comprehensive medication reviews (CMR).
  2. Patient Counseling and Education:
    • Provide patients with clear and comprehensive information on their medications, including proper usage, potential side effects, and drug interactions.
    • Educate patients on lifestyle and dietary modifications to enhance medication efficacy.
    • Address patient concerns and questions regarding their medications.
  3. Collaboration with Healthcare Team:
    • Work closely with physicians, nurses, and other healthcare professionals to develop and implement individualized treatment plans.
    • Participate in interdisciplinary team meetings to discuss patient care and optimize treatment outcomes.
    • Serve as a resource for healthcare providers regarding medication management and best practices.

 

  1. Regulatory Compliance and Documentation:
    • Ensure compliance with all federal, state, and local regulations governing pharmacy practice.
    • Maintain accurate and complete records of all prescriptions and patient interactions.
    • Conduct regular audits and reviews to ensure adherence to regulatory standards.
  2. Quality Improvement and Patient Safety:
    • Monitor and report adverse drug reactions and medication errors.
    • Implement quality improvement initiatives to enhance pharmacy services and patient safety.
    • Participate in continuing education and professional development to stay current with best practices and advancements in pharmacy.
  3. Community Outreach and Education:
    • Engage in community outreach activities to promote health and wellness.
    • Provide educational sessions on medication management and preventive healthcare to community groups

    Ministry Leave Opportunity: After 1-year of full-time service employees have the opportunity for paid leave up to 2 weeks for a short-term ministry away from CRHM.
