Job Summary:
Join our client's team as a Physical Therapist for PACE, responsible for evaluating and assessing participants to determine mobility levels and physical conditions. Develop individual and group exercise programs while overseeing functional maintenance programs. Provide comprehensive physical therapy for participants and ensure therapy aligns with industry standards. 

 

Responsibilities:

  • Assess participant data, prioritize needs, and set goals for rehabilitation.
  • Develop care plans integrating interdisciplinary needs and participant/family input.
  • Administer rehabilitation treatments following prescribed standards.
  • Educate participants, family, and staff on rehabilitation needs and interventions.
  • Supervise program aides and staff implementing therapy programs.
  • Participate actively in Quality Improvement processes and interdisciplinary team collaboration.
 

Qualifications:

  • Bachelor’s or Master’s degree in Physical Therapy.
  • Licensed Physical Therapist in the required state.
  • Knowledge of clinical competency with the geriatric population.
  • Experience working with the elderly population preferred.
  • Reliable transportation, valid driver’s license, and liability auto insurance.
  • Clear background check and up-to-date immunizations.
  • Ability to work effectively in a team environment and communicate openly with interdisciplinary team members.
