WHAT YOU CAN EXPECT:

  • Collaborate with multidisciplinary teams to evaluate and assess patient's medical and physical needs.

  • Refer patients to appropriate services if patients are identified with depression, anxiety, grief or mental illness.

  • Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy.

  • Assist patients and their caregivers in identifying goals of care and assist with unmet needs through linkages to community resources.

  • Perform other duties as assigned

  • Comply with all policies and standards

QUALIFICATIONS

  • Master's degree in Social Work required

  • Indiana LSW required

  • 1 years of Social Work experience preferred
