Psychiatric Nurse Practitioner

Mosaic Health, a network of Federally Qualified Health Centers in Western and Central New York, is seeking a Psychiatric Nurse Practitioner to work with our integrated care team of medical, dental, and behavioral health staff. The Psychiatric Nurse Practitioner will serve the psychiatric medication management needs of our primary care patients.

This opportunity can be filled on a full or part-time (at least 24 hours per week) basis with flexibility to utilize telehealth while working from one of sites in Rochester, Rushville, Utica, or Ilion while proving care to our established primary care patients across our service area. In addition, there is a dedicated Behavioral Health Medical Assistant to coordinate all administrative aspects of the program.
