Description

Responsible as a member of the interdisciplinary team too provide medical social services to the patient and family in the home.  This will be consistent with Hospice policies and procedures.


 


Qualifications

Qualifications and Experience



  • Graduate of a school of social work approved by the Council of Social Work Education with a master’s degree (MSW) or has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other field related to social work and will be supervised by an MSW.  (Diploma or transcripts to be provided to agency)

  • If education level is baccalaureate then employee must have one year of experience in a healthcare setting.

  • Prior Home Care experience preferred but not necessary.

  • Demonstrate experience in working with patient/families.

  • Demonstrates good verbal and written communication and organizational skills.

  • Licensure by the state.

  • Must maintain current drivers license, auto insurance, CPR card and TB test on file.

  • A criminal background check must be performed before hire.

  • Complete and pass skill evaluation test.


Essential Duties and Responsibilities *



  • Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.

  • Carries out social evaluations and plans intervention based on evaluation findings.

  • Maintains clinical and progress notes on all patients referred to social work.

  • Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.

  • Works with family as needed.

  • Consults and coordinates services with other disciplines.

  • Provides information to patients or families/caregivers and community agencies.

  • Serves as liaison between patients or families/caregivers and community agencies.

  • Maintains collaborative relationships with organization personnel to support patient care.

  • Maintains and develops contacts with public and private agencies as resources for patient and organization personnel.

  • Participates in; the development of the total plan of care in consultation with the physician and other care team member and case conferences as required.

  • Assists physician and other team members in understanding significant social and emotional factors related to health problems.

  • Participates in discharge planning.

  • Participates in in-servicing programs.

  • Other duties as delegated by the Clinical Director/Supervisor.
