MAJOR FUNCTION:
Provides a variety of social service needs to the medical patient and caregivers as they transition to the appropriate level of care. Responsibilities include completing psychosocial assessments for specific patient populations including dialysis, mother baby and oncology, collaborating with the interdisciplinary team in discharge planning for high-risk patients. Participates in interdisciplinary rounds; educates patients and provides them as well as their caregivers with appropriate community resources and agencies to help ensure safety upon discharge. The master's Prepared Social Worker will function within the guidelines of the National Association of Social Work and ensure responsibility for continued professional and personal growth.
